Paid Time Off (PTO) Regular full-time and regular part-time employees After completion of 90 day evaluative period. LCMH Combines vacation, holidays and 24 extra hours to use for short term illness/injury or other personal time off.

Extended Illness (ELB) Regular full-time After completion of 90 day evaluative period. LCMH Accrue 9 days 72 hours per year or 2.7692 hrs per pay period. Used when hospitalized, for out-patient surgery and when time off exceeds 24 hours for short term illness/injury.
Regular part-time After completion of 90 day evaluative period.   Accrue based on hours paid in the pay period.
Health Insurance Regular full-time and regular part-time 1st of month after hire or during annual enrollment period in May. LCMH/ Employee Coverage with Employee Benefit Services includes $2,000,000 per person lifetime maximum payable on the medical and dental plan.
Dental Insurance Regular full-time and regular part-time Effective 90 days after enrollment. LCMH/ Employee Calendar year maximum per individual $1,100.00
Life Insurance Regular full-time employees 1st of month after completion of 90 day evaluative period. LCMH 1 - 1/2 times annual salary up to a maximum of $50,000 insurance.
Long Term Disability Regular full-time employees 1st of month after completion of 90 day evaluative period. LCMH 50 % of base salary beginning on the 91st day of disability.
Retirement All employees who are at least 21 years of age and who worked 1000 hours during the calendar year End of calendar year after completion of 12 months employment LCMH Amount contributed per year based on percentage of employees gross yearly wages. 20% vesting each year beginning with the 3rd year. Fully vested after 6 years.
Flexible Benefits Regular full-time and regular part-time employees Upon hire and during annual enrollment period. Employee contributes money for accounts. Receives tax savings, includes 2 spending accounts: dependent care and unreimbursed medical and dental expenses. Unused contributions each year forfeited.

Tax Deferred Annuity All employees Upon hire Employee Tax deferred savings through payroll deduction designed primarily for retirement annuity.
Tuition Reimbursement Regular full-time and regular part time employees. After completion of 90-day evaluative period LCMH Employee must sign contract to work 2 years once degree earned. Must receive prior approval and complete course with a C grade or better. 100% paid for F/T and 50% for P/T up to 9 credit hours. Area of study must be related to employment at LCMH.
Child Care Assistance Regular full-time and regular part-time employees. Upon hire LCMH $20/wk employee if regular FT $10/wk employee if regular PT For licensed child care facilities.
Bereavement Leave Regular full-time employees Upon hire LCMH Base pay for up to 3 regular scheduled work days (24 hours) for immediate family only. Grandparents 1 day ( 8 hours).
Worker's Compensation All Employees Upon hire LCMH Protection for job-related accidents and injuries according to State of Louisiana Law.
Cafeteria Discount All Employees Upon hire LCMH/ Employee 50% discount on purchase of food or drink. Must be wearing hospital ID Badge.
Pharmacy Discount All Employees Upon hire LCMH/ Employee Drugs at cost plus 10%. Convenience of in-house pharmacy.
Outpatient Discount Regular full-time and regular part-time employees Upon hire LCMH/ Employee 50% discount on hospital outpatient services after insurance payment. Day surgery is not included.
Family and Medical Leave (FMLA) Regular full-time and regular part-time employees After one year of employ- ment during which at least 1000 hours have been worked. Not Applicable Up to 12 weeks of leave granted with medical certification in accordance with federal guidelines.
Jury Duty All Employees Upon hire LCMH Full base salary while on jury duty. You must then endorse your jury duty check over to the Hospital.
Credit Union/ SWLA Credit Union All Employees Upon hire Not Applicable Federally insured savings and low interest loans by payroll deduction.
Service Awards All Employees At five-year intervals LCMH Special recognition consisting of banquet and award.
Employee Health All Employees Upon Hire LCMH Hepatitis vaccinations given, as well as annual TB testing.
Direct Deposit All Employees Upon Hire LCMH You can have your paycheck direct deposited to your checking or savings account.
